Frequently Asked Questions

SAP Evaluation FAQ

Straight answers to the questions drivers ask most often about the DOT Return-to-Duty process. If you have a question that isn’t covered here, your SAP can answer it during your evaluation.

What happens after I pay the $95?

You will be directed to complete the Pre-Assessment Questionnaire online. Once that is done, you can schedule your virtual SAP evaluation. Most appointments are available within the same week. Your SAP guides you through every step.

How long does the whole RTD process take?

The Return-to-Duty process typically takes 2 to 6 weeks from start to finish, depending on the SAP's recommendations. The initial evaluation usually happens within 1 week of payment. After completing any recommended education or treatment, you complete a follow-up evaluation before being cleared for return-to-duty testing.

Will my employer see what I tell the SAP?

Your evaluation details are confidential. Your SAP only shares compliance documentation required by DOT regulations (49 CFR Part 40): confirmation you completed the evaluation and follow-up, and the final return-to-duty recommendation. Personal details discussed in the evaluation stay private.

Can I reschedule my evaluation if something comes up?

Yes. Reschedule directly with your SAP. Most SAPs offer flexible scheduling including evenings and weekends. There is no penalty for rescheduling, but try to give at least 24 hours notice when possible.

What if the SAP recommends treatment I cannot afford or fit into my schedule?

The SAP's recommendations are based on what is needed for safe return-to-duty. If a recommendation creates a hardship, talk to your SAP about alternatives. Many programs offer payment plans, online formats, or different intensities of treatment. Your SAP is your guide.

What if I "fail" the evaluation?

There is no pass or fail. The evaluation determines what you need to safely return to duty. If your SAP recommends education or treatment, completing those steps is what gets you back to work. The follow-up evaluation confirms you have met the recommendations and are ready for the return-to-duty test.

Why is the evaluation virtual? Is that allowed?

Yes. Per 49 CFR section 40.281 as amended in May 2023 (88 FR 27596), DOT SAP evaluations may be conducted remotely using real-time audio and video. Virtual evaluations meet all federal requirements and give you faster access to qualified SAPs across the country.

What is the difference between the $95 Pre-Assessment and the SAP Fee?

The $95 Pre-Assessment Questionnaire is provided to your SAP and gives them the needed information to begin their portion of the Return-to-Duty process with you.

The SAP's fee (up to $500) is paid directly to the SAP and generally includes the initial evaluation, follow-up evaluation, and Return-to-Duty recommendation review.

Every situation is different. Depending on the SAP's recommendations, additional education, treatment, or follow-up services may be required. Any fees associated with those services are separate and are determined directly between you and your SAP.

Total Return-to-Duty process cost as listed above: up to $595.
